Telehealth Physical Therapist (PT) Primarily supporting Keizer Nursing & Rehabilitation Center (Keizer, OR) and French Prairie Nursing & Rehabilitation Center (Woodburn, OR) Compensation: $45–$55/hr (DOE) Schedule: PRN Work Location: Remote Are you ready to make a meaningful impact while advancing your career? What You'll Do • Provide physical therapy services via telehealth to residents in our Oregon facilities • Conduct evaluations and develop individualized treatment plans • Guide residents through exercises, functional training, and pain management strategies virtually • Educate residents and families on safe use of equipment and exercise techniques • Document patient progress accurately and on time • Collaborate remotely with on-site nursing, therapy, and interdisciplinary care teams What We're Looking For • Active Physical Therapist license in Oregon (required) • Graduate of a CAPTE Accredited Physical Therapy Education Program • Experience in post-acute care or similar healthcare environment preferred • Strong communication and collaboration skills with a focus on patient care • Ability to develop and implement effective treatment plans to meet patient goals • Completes documentation and billing accurately and on time • Problem-solving skills and willingness to meet productivity benchmarks Benefits That Put You First • Competitive pay • Medical, Dental, and Vision insurance for you and your family • Company-paid life insurance with optional additional coverage • 401(k) with company contributions • Generous Paid Time Off (PTO) and paid holidays • Tuition reimbursement & CEUs • Annual licensure reimbursement (for full-time employees after one year of employment) • Unlimited employee referral bonuses Note: Benefit eligibility may vary for part-time and PRN employees.
